Physical Therapist- PT Home Health, Phoenix, AZ - Local or Travel 13 weeks Shift: M-F, 8am - 5pm, 40 hours per week On-call info: On-call required if requested- on-call needs are subject to change based on staff and branch needs Productivity Units: 30 units Qualifications: HCHB, Oasis, and HH experience required Skills Current State License as a Physical Therapist - Required Proof of current CPR certification - Required Current driver's license. Must have reliable transportation and insurance - Required 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required Previous home care experience - Preferred HCHB EMR preferred - Education Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - RequiredLocal: Taxable pay: $61/hr Total Gross pay : $2440/week OT& Holiday rate: $91.5/hr Travel: Taxable pay: $37/hr OT& Holiday rate: $55.5/hr
