Constant Care Health Services LLC - Constant Care Health Services is a Licensed Home Care Agency located in the Bronx. We are currently seeking a Part Time RN Field Nurse to join our Team!This position requires expertise in the areas of patient and family assessment, determining appropriateness of and acceptance for home health services, coordination of services given by other health care providers and knowledge of state regulations.Qualifications:· Currently Licensed and Registered in the State of New York as a Registered Professional NurseBachelor's Degree in Nursing preferredOne-two years nursing experience in acute care settingOne year experience in home careKnowledge in EMRDriver's license and a vehicle requiredRESPONSIBILITIES:Provides assessment of newly referred clients, assesses health needs and eligibility for home careProvides patient and family teaching prior to discharge and on an on-going basisPerforms patient assessments in the home and document in EMR timely.Provides skilled nursing on assigned clients according to medical orders, and policies and proceduresCommunicates with referrers/discharge planners to review eligibility for agency services and to develop and coordinate the plan of careAssists referring agents in alternate plans for ineligible clientsDevelops and implements treatment plans in conjunction with patient's authorized medical practitionerCreate HHA plan of care and discuss the same with the assigned aideSupervise all involved disciplines in the plan of treatmentDo performance eval of the assigned aides, supervision of the aides and their competenciesOther duties as deemed appropriatePerforms other nursing activities as directedHP
