Outpatient Case Manager: Evening & Weekends
Hushabye Nursery is an Arizona nonprofit organization with a mission to 'embrace substance exposed babies and their caregivers with compassionate, evidence-based care that changes the course of their entire lives.' The organization was founded by Tara Sundem, a neonatal nurse practitioner who witnessed firsthand the challenges of newborns exposed prenatally to opioids and those families trying to care for them. Hushabye Nursery's innovative model offers a safe and inclusive space where mothers, family members and babies - from conception through childhood - can receive integrative care and therapeutic support that offers each child the best possible life outcomes. Programs will include prenatal and postpartum support groups, inpatient nursery services, and outpatient therapies. POSITION OVERVIEW: The Case Manager provides comprehensive, client-centered services to pregnant and parenting individuals receiving outpatient substance use support. This role is responsible for assessment, treatment planning, care coordination, trauma-informed service delivery, progress monitoring, and discharge planning. Case Managers work collaboratively with a multidisciplinary team to support recovery, family stability, and healthy outcomes for parents and families. Case Managers are expected to meet the productivity standards outlined by their position in accordance with program and payer requirements.DUTIES & RESPONSIBILITIES:Clinical and Case Management ServicesConduct comprehensive biopsychosocial assessments to identify client needs across behavioral health, physical health, housing, employment, legal, and social determinants domains.Develop, implement, and update individualized treatment plans in collaboration with clientsProvide individual case management and skills training sessionsFacilitate group sessions as assignedCoordinate and document referrals, monitor follow-through, and advocate for access to support (medical providers, other behavioral health providers, child welfare, vocational, housing, transportation, community resources etc).Monitor client progress toward treatment goals and document outcomesParticipate in multidisciplinary treatment team meetings and transition/discharge planning.Provide education related to substance use recovery, parenting, prenatal/postpartum health, and life skillsSupport client engagement, retention, and adherence to treatment recommendationsDocumentation and ComplianceComplete all clinical documentation accurately, thoroughly, and within 24 hours of serviceMaintain timely and accurate clinical documentation in compliance with organizational policies and regulatory standards (i.e., AHCCCS, ADHS, and CARF)Ensure documentation supports medical necessity and billable service requirementsMaintain compliance with confidentiality, ethical standards, and professional boundariesParticipate in audits, chart reviews, and quality improvement activitiesDischarge Planning and Continuity of CareDevelop discharge and aftercare plans in collaboration with clientsCoordinate referrals to ongoing treatment, recovery supports, and community servicesEnsure continuity of care and appropriate transitions upon dischargeTeam Collaboration and Professional ResponsibilitiesParticipate in supervision, team meetings, and case conferencesCollaborate with interdisciplinary staffEngage in ongoing training and professional developmentContribute to a trauma-informed, recovery-oriented, and family-centered program cultureProductivity ExpectationsStaff in this role are expected to meet the productivity standards outlined by their position in accordance with program and payer requirements.Balance direct service delivery with required documentation and administrative tasksProactively manage schedule to meet productivity standards while ensuring quality careRequirementsKNOWLEDGE, SKILLS, AND EXPERIENCE:Skills and CompetenciesStrong clinical engagement and rapport-building skillsEffective group facilitation and individual session abilitiesExcellent time management and organizational skillsClear, accurate, and timely documentationAbility to manage caseloads and productivity expectationsProfessional boundaries and ethical decision-makingCultural humility and sensitivity to diverse family systemsWork Environment and ExpectationsOutpatient clinical setting serving pregnant and parenting clientsMay include coordination with external systems such as child welfare, healthcare providers, and courtsRequires flexibility, professionalism, and a commitment to client-centered careThis position plays a critical role in supporting recovery, parenting capacity, and long-term family stability for clients served by the program.EDUCATION (MINIMUM REQUIREMENTS) and Experience:Required:Bachelor's degree in behavioral health, social work, human services, or related field. Equivalent experience as outlined below can serve in lieu of a bachelor's degree.3 years of experience in Social Work or another related field.Experience working in substance use treatment or behavioral health settingsKnowledge of recovery-oriented, trauma-informed, and family-centered careAbility to meet documentation, billing, and productivity requirementsPreferred:Experience working with pregnant and parenting populationsCase management or counseling certification or licensure (as applicable)Familiarity with Medicaid/AHCCCS or similar payer systemsLived experience relevant to recovery or parenting (valued but not required)PHYSICAL/MENTAL DEMANDS & ENVIRONMENT:PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
