MDS Coordinator

Responsibilities:Coordinate the Minimum Data Set (MDS) process for residents in accordance with regulatory guidelinesConduct comprehensive assessments of residents' health status and care needsCollaborate with interdisciplinary teams to develop individualized care plansEnsure accuracy and completeness of MDS assessments and documentationStay updated on changes in regulations and requirements related to MDSTrain and educate staff on MDS processes and documentation standardsParticipate in quality improvement initiatives and audits.Communicate effectively with residents, families, and healthcare professionals Requirements:Current licensure as a Registered Nurse (RN) or LPNPrevious experience as an MDS Coordinator preferred.Knowledge of MDS process and regulationsStrong attention to detail and organizational skillsExcellent communication and interpersonal abilitiesAbility to work collaboratively in a team environmentProficiency in electronic health record systems preferred
