Job Responsibilities: The Physical Therapist is responsible for the assessment, delivery, documentation, modification and evaluation of Physical Therapy services based on the patient's needs and the prescription of the attending physician in accordance with the Physical Therapy Act and agency policies and procedures. Job Qualifications: Applicant must be a graduate of an accredited Physical Therapy Program and, Licensed by the State of Kentucky. Must have current driver's license, the ability and willingness to travel, and reliable transportation. Position requires the physical ability to perform the essentials functions of the job.
