manage a wide array of acute and chronic health issues, perform physical examinations, order and analyze diagnostic tests, and create patient management strategies.Collaborative Teamwork: Work alongside a Patient Aligned Care Team (PACT) that includes an RN care manager, LPN, clinical pharmacist, psychologists, and social workers to provide integrated, comprehensive care.Education & Mentorship: Guide and mentor medical students, residents, and other healthcare learners, nurturing the next generation of medical professionals.Integrated Support Services: Operate within a system students.Adhering to current standards for medical record documentation and supervision of residents/medical students.Providing appropriate care for Veterans across all age groups, considering their physical, psychosocial, spiritual, and cultural needs.Conducting thorough health assessments, determining diagnoses and healthcare needs, and developing care plans based on effective therapeutic practices, consultations, patient education, and established guidelines.Demonstrating the ability to coordinate patient care through management techniques within the assigned Patient Aligned Care Team (PACT) and implementing similar functionalities in other PACTs.Applicants must be U.S. citizens, hold an M.D. or D.O. degree, possess a full, active, and unrestricted medical license, and have at
