As an Engagement Specialist, you'll be the first point of contact for patients-onboarding, scheduling, and keeping them connected through high-volume, relationship-focused outreach. This role is perfect for someone organized, motivated, and skilled at building trust while helping patients navigate their healthcare journey.Position Details:Location: Remote, work from home. This is currently a Remote position. However, we value in-person patient care and may transition to a hybrid-remote model in the future.Schedule: Monday-Friday, full-time schedule with the ability to accommodate patient availability- flexibility is key. No nights, No weekends, No on call. Daily may start time between 7:00-8:00 AMHome environment must include a quiet, private office space with high-speed internet (Ether cable may be required) to support efficient use of all programs.Engagement Specialist Responsibilities:Facilitate CHE completion in nephrology offices, dialysis clinics, or via telehealth; coordinate with Nurse Practitioners and clinics to prepare for in-person appointmentsPerform outbound calls to patients, caregivers, and clinics for CHE scheduling, reminders, and rescheduling; schedule telehealth CHEs for patients unable to attend in personWelcome new patients to the program and collect any new concerns from existing patients; escalate to RN or Nurse Practitioner as appropriateMaintain accurate documentation and communicate patient updates to the care teamDemonstrate exceptional interpersonal skills to build trust, influence patient participation, and overcome barriers to engagementOrganize outreach efforts by urgency and importance to maximize patient impactCare Coordinator Responsibilities:(May represent some role responsibilities)Coordinate care for patients by ensuring access to eligible resources/programs and assisting with referrals and provider appointmentsPerform outbound calls and respond to inbound calls from patients, providers, and Village resourcesComplete non-clinical screenings and pathways under the guidance of the Manager; escalate to Integrated Care Nurse as neededAssist the clinical team with moving patients through the continuum of care by completing tasks as directedIdentify eligible resources for members and collaborate with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to help members enroll in programsCommunicate directly with patients in a timely and professional mannerMaintain accurate, complete, and confidential documentationDemonstrate exceptional two-way communication, both oral and writtenAct in alignment with DaVita's mission and values to deliver high-quality patient careQualifications:High School Diploma or GED required; Associate's or Bachelor's Degree preferredPrevious IKC experienced required2+ years professional experience in customer service, patient-facing, or telephonic outreach roles preferredPrior healthcare experience preferredIntermediate proficiency in Microsoft Word, Excel, PowerPoint, and OutlookStrong verbal and written communication skills, active listening, and motivational engagement techniquesHighly organized, with excellent time management and follow-throughAbility to work both autonomously and collaboratively with care team membersHome workspace must provide a quiet, private environment with reliable high-speed internetWhy You'll Love Working Here:Directly impact patient care and engagement for complex chronic conditionsBe part of a mission-driven, collaborative care teamFlexible hybrid schedule with opportunities for professional growthSupportive environment that values innovation, communication, and patient-centered careJoin our team and help patients navigate their care journeyWhat We'll Provide:More than just pay, our DaVita Rewards package connects teammates to what matters most.
