Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC)

*Job Summary* We are seeking a *Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC)* to provide high-quality behavioral health services within our *Intensive In-Home (IIH)* and *Community Support Team (CST)* programs. The clinician will deliver *individual and family counseling*, complete *Clinical Comprehensive Assessments (CCA)*, contribute to *person-centered treatment planning*, and collaborate with multidisciplinary team members to support individuals and families in the community. The ideal candidate is compassionate, organized, and comfortable working with high-needs populations in home and community-based settings. *Essential Duties & ResponsibilitiesClinical Services* * Provide *individual, family, and group counseling* to children, adolescents, and/or adults as appropriate * Conduct *Clinical Comprehensive Assessments (CCAs)* in compliance with NC Medicaid and LME/MCO standards * Develop, review, and update *Person-Centered Plans (PCPs)* and treatment goals * Deliver *Intensive In-Home therapy* to youth and families with complex emotional and behavioral needs * Provide *clinical oversight and therapeutic interventions* in collaboration with CST staff * Utilize evidence-based and trauma-informed practices (CBT, MI, strengths-based approaches, etc.) *Team Collaboration & Oversight* * Collaborate with *CST team members*, Qualified Professionals (QPs), Peer Support Specialists, and care coordinators * Participate in *team meetings, case staffing, and clinical consultations* * Provide guidance and clinical input to support staff as needed * Coordinate care with schools, medical providers, DSS, juvenile justice, and other community partners *Documentation & Compliance* * Complete all clinical documentation accurately and timely, including: * CCAs * Progress notes * Treatment plans * Clinical reviews and updates * Ensure services meet *NC Medicaid*, *DMH/DD/SAS*, and *LME/MCO* requirements * Maintain client confidentiality and ethical standards in accordance with licensure and agency policy *Required Qualifications* * Active, unrestricted *LCSW or LPC license in North Carolina* * Masters degree in Social Work, Counseling, or a related behavioral health field * Experience providing *clinical assessments and counseling services* * Knowledge of *Intensive In-Home and/or Community Support Team services* preferred * Ability to work independently in *home and community settings* * Strong clinical judgment, communication, and organizational skills * Valid drivers license, reliable transportation, and current auto insurance * Ability to pass background check and credentialing requirements *Preferred Qualifications* * Previous experience with *IIH, CST, or Medicaid-funded services* * Experience working with *children, adolescents, families, and high-risk populations* * Familiarity with trauma-informed care, substance use, and co-occurring disorders * Experience with electronic health records (EHR) *Schedule & Work Environment* * Flexible scheduling including afternoons/evenings as needed * Community-based work in *Union County and surrounding areas* * Combination of in-person and approved telehealth services (when applicable) Pay: From $30.00 per hour Work Location: Hybrid remote in Monroe, NC 28110 by Jobble
