Job Description
Job Title: Social Worker
Access to Care
At initial and annual intakes identify medical coverage, household income and potential barriers to care (housing, transportation, financial, mental health, substance use, etc.)
Assist in the enrollment and maintenance in any primary (e.g. Medicaid) or supplemental (e.g. OHDAP) coverage as needed to cover all medical expenses (i.e. benefits assistance)
Make reminder calls for medical and mental health appointments, assisting patient to overcome any barriers to keeping appointment
Verify and document patient coverage for on-site and referral appointments
Make referrals for any issues that will impede access to care
Participate in outreach activities
Retention in Care
Assist patient in overcoming any barriers to returning to care (e.g. transportation solutions, mental health referrals, etc.)
Use self-management model to assist patients in goal setting that will contribute to retention in care, reduce barriers to care (e.g. drug rehab, getting a driver’s license)
Optional (strongly encouraged) attendance at monthly Ministry Café
Utilization/Quality of Care
Review charts/case notes before clinic so that all areas of concern are addressed at visit
Conduct mental health assessments and refer patients to counseling and/or substance use programs as indicated
Make follow up calls to patients that have missed their appointments.
Provide transportation assistance to appointments for clients that need a staff person present.
Use self-management model to assist patients in goal setting that will contribute to improved quality of medical care & quality of life
Collaborate with clinic staff, Ryan White Part B case managers and outside agencies
Participate in Quality activities as part of interdisciplinary staff meetings
Fulfill medical records request for SSA
Complete Physician Reports
Coordinate patient education
Obtain patient’s proof of incomes
General
Maintain patient confidentiality
Document/chart patient progress immediately and accurately
Perform work in a timely and productive manner
Take action to improve knowledge, skills and performance based on feedback or on self-identified needs; maintain professional license
Maintain patient documentation
Document patient contact
Document pertinent information from other agencies/providers.
Maintain daily log
Report job-related or patient-related concerns to Clinic Director
Assist with Ministry fundraising when asked
Performs other duties as necessary
