This full-time opportunity is ideal for professionals who are passionate about helping patients regain independence, improve functional abilities, and enhance their overall quality of life through individualized therapy interventions. As an Occupational Therapist in the SNF setting, you will work with patients recovering from illness, injury, surgery, or age-related conditions, supporting their ability to perform daily activities safely and independently. You will collaborate closely with interdisciplinary healthcare teams to deliver high-quality patient-centered care. Responsibilities: Evaluate patients’ physical, cognitive, and functional abilities to determine therapy needs Develop and implement individualized treatment plans focused on improving independence and daily functioning Provide interventions addressing mobility, self-care activities, upper extremity function, and adaptive strategies Train patients and caregivers on equipment use, safety techniques, and home modifications when appropriate Monitor patient progress and adjust treatment plans based on outcomes and goals Maintain accurate documentation and comply with facility, state, and federal guidelines Collaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and interdisciplinary teams Participate in discharge planning and recommendations for continued care Requirements Qualifications: Active Occupational Therapist (OT) license required Previous experience in a Skilled Nursing Facility (SNF) setting preferred, but qualified candidates with related experience are encouraged to apply Strong clinical reasoning, assessment, and patient care skills Excellent communication and interpersonal abilities Ability to work independently while contributing to a collaborative team environment Commitment to delivering compassionate, high-quality therapy services Benefits Why Consider This Opportunity?
