Advance Practice Provider (Nurse Practitioner or Physician Assistant) - Full-Time Clinical Hospital

Working closely with physicians and multidisciplinary care teams, the APP ensures continuity of care from hospital encounters through outpatient communication and coordination.This role blends clinical autonomy with strong team support and offers meaningful involvement in quality improvement, patient experience, and care coordination initiatives.Key ResponsibilitiesProvide comprehensive patient care including histories, physical exams, diagnoses, and evidence-based treatment plansCounsel and educate patients and families using clear, compassionate, and culturally sensitive communicationCoordinate care across inpatient and outpatient settings to support continuity and optimal outcomesOrder and interpret appropriate labs, imaging, and consults with attention to quality, safety, and costCollaborate within a Care Team model to support high-risk patients, care planning, and self-managementParticipate in quality improvement initiatives, peer reviews, and workflow enhancementsMaintain timely, accurate documentation in the electronic medical record (EMR)Required QualificationsEducation & LicensureNurse Practitioner: Master's degree from an accredited nursing programPhysician Assistant: Graduation from an accredited PA programActive Maryland license (or ability to obtain)Current DEA license and Maryland Controlled Dangerous Substance (CDS) licenseCurrent BLS certification (American Heart Association or approved provider)Unrestricted license with no history of suspension or revocationEligible for participation with all payersSkills & ExperiencePrior experience in Gastroenterology or adult primary care preferredStrong clinical judgment and commitment to evidence-based practiceProficiency with electronic medical records (EMR experience preferred)Excellent organizational, time-management, and multitasking abilitiesStrong interpersonal, communication, and problem-solving skillsAbility to work independently while thriving in a team-based environmentCaring for you as you care for the CommUNITYFrederick Health offers a comprehensive and affordable benefits package.Health, Dental and Vision insurance are offered the 1st of the month after 30 days of employment to all employees hired to work at least 20 hours per week and we offer multiple plans to best meet your and your family's needs.Life insurance, Short-Term Income Replacement and Long-Term Disability are employer paid for eligible employees.Frederick Health offers a robust Paid Time Off program for eligible employees.Our 403B retirement plan helps you save for your retirement and includes an employer match to eligible employees.
