Community Health Worker- Patient Navigator

The CHW-PN will undergo two training courses upon hire. First the CHW training and Second American Cancer Society LION Navigation Certification Program. They will be responsible for providing administrative oversight to a project design focused on Improving Quality, Improving Messaging, and promoting breast health services to target groups, and working with breast health patients one-on-one to provide navigation to specialty services along the care continuum. He/she will ensure the quality and success of special programs and will be tasked with monitoring overall program progress, providing high-quality coordination and facilitation, customer service, and presenting reports.QUALIFICATIONS: High School Diploma, GED Willing to undergo specialized training courses (CHW and ACS LION). Willing to commit to a one-year project plan. Willing to work under the terms of a grant-funded position. Excellent interpersonal and organizational skills. Ability to manage multiple tasks and projects. Demonstrated interest and/or strong understanding of current and emerging public health issues. Knowledge of local area resources is a plus. Bilingual – English/Spanish is required. ESSENTIAL DUTIES AND RESPONSIBILITIES: Partner with other health center staff to enact a defined project plan that will result in increase quality and increased patient navigation services. Refine procedures and processes related to breast health care. Will work closely with the Community Health Dept. to analyze the program, and future needs and will make recommendations for improvement and implementation. Assess SBCHC data collection and reporting process. Establishes priorities, and monitors and evaluates the effectiveness and efficiency of current health center programs. Makes appropriate recommendations for improvement. Coordinating program events and meeting logistics including education materials related to breast health care. Partner with interdisciplinary medical team to identify patients eligible for special programs. Maintains communication with patients and conducts patient follow-up to ensure that patients are accessing needed services/programs. Prepares and sends official correspondence related to special programs. Maintains files of program-related documents, letters, patient testimonies, etc. Update and maintain breast health care program material. Produce culturally and literacy-appropriate program materials. Participates in all safety programs which may include assignment to an emergency response team. Perform other duties as assigned.
