Job SummaryProvider Performance Liaisons (PPLs) will develop partnerships with providers to close identified gaps in care and achieve targeted thresholds in Alternative Payment Model (APM) programs and customized plans. The PPL will work closely with providers, community-based organizations, and internal partners to implement interventions that impact improved quality outcomes for our membership.Essential FunctionsParticipate in the annual and customized development of APMsAssist in prioritizing opportunities for improved quality outcomes for the provider’s attributed populationParticipate in population health quality assurance and improvement activities with the provider and community partnersEnhance effectiveness of provider’s population health interventions through the use of research and QI science.Participate and support Quality Withhold efforts as directedCollaborate with Population Health Analytics team in coordinating the exchange of clinical information and member/population level dataAssist in implementation of state-wide plans in collaboration with leadership, population health analytics team, Provider Network Performance Team, Health Equity, and Quality.Engage in onboarding activities specific to APM support and interventions targeting contracted quality metricsStrategize with the provider to develop and enhance the practice’s engagement strategies for individual member populationsProvide education on available resources to providers to facilitate population health managementDevelop, educate, implement, and serve as a consultant regarding CareSource’s care management programsSupport local presence by participating in events, meetings and opportunities in the communityCollaborate to identify areas of opportunity to improve capabilities and partnership processesPerform any other job duties as requestedEducation And ExperienceBachelor’s Degree in social work, nursing, public health, or related field or equivalent years of relevant work experience is preferredMinimum of three (3) years of experience in a Provider-facing setting or role is requiredExperience working with data and data analytics is preferredCompetencies, Knowledge And SkillsAnalytic skills for solving problemsWord, Excel, and/or Access skills are required; SQL and/or SAS experience preferredAbility to operate a smart phone, tablet, or other mobile communication devices to ensure productivity and ability to perform essential functionsAbility to act with diplomacy and sensitivity to cultural diversityStrong interpersonal skills and high level of professionalismAbility to maintain confidentiality and act in the company’s best interestAbility to work independently and within a team environmentKnowledge of quality management/statistical reportingChange resiliencyExcellent conflict resolution skillsExceptional emotional intelligenceCustomer service orientedCritical listening, thinking skills, and verbal and written communication skillsExcellent written and verbal communication skillsKnowledge of local, state and Federal laws and regulationsKnowledge of healthcare resources, benefits and entitlements in the local areaAbility to develop, prioritize and accomplish goalsProven track record for improving processes to make things easier for those you have servedLicensure And CertificationCurrent unrestricted license in field of practice including (if applicable): Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), Professional Counselor (PC), or Licensed Professional Clinical Counselor (LPCC), or Registered Nurse (RN) license in the State of Service is preferredEmployment in this position is conditional pending successful clearance of a driver’s license record check. A valid driver’s license, car, and insurance are necessary for work related travelRequired to use general office equipment, such as a telephone, photocopier, fax machine, and personal computerFlexible hours, including possible evenings and/or weekends as needed to serve the needs of our health partnersCompensation Range$72,200.00 - $115,500.00 CareSource takes into consideration a combination of a candidate’s education, training, and experience as well as the position’s scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
