The LPN works under the supervision of the Director of Nursing and collaborates with the interdisciplinary care team to ensure quality resident care.ResponsibilitiesCollect and document admission information including medical records and insurance detailsRecord resident health information including vital signs and temperatureAdminister medications and injections as prescribedProvide wound care and treatment of pressure injuries as neededSupervise Certified Nursing Assistants (CNAs) when assignedAssist residents with personal hygiene and daily care needsMonitor and document residents' intake and outputReport changes in resident condition to appropriate clinical leadershipQualificationsActive New York State Licensed Practical Nurse (LPN) license requiredMust be in good standing with the State RegistryPrior long-term care or rehabilitation experience preferredStrong clinical and communication skills
