Physical Therapist (PT) / Occupational Therapist (OT) – Home Health
Join Vertus Healthcare and provide in-home rehab care that helps patients regain independence and stay safe at home. We’re looking for licensed PTs/OTs to deliver personalized care, educate patients and caregivers, and collaborate with a supportive team.
We are seeking a licensed Physical Therapist (PT) / Occupational Therapist (OT) to provide high-quality, patient-centered rehabilitation services in the home setting. The ideal candidate will evaluate, treat, and educate patients to improve functional independence, mobility, and overall quality of life while adhering to agency policies and regulatory guidelines.
Key Responsibilities
Conduct initial and ongoing patient assessments in the home setting.
Develop individualized treatment plans based on patient goals and clinical needs.
Implement evidence-based interventions to improve strength, mobility, balance, ADLs, and safety.
Educate patients and caregivers on exercises, safety, adaptive equipment, and fall prevention.
Collaborate with nurses, social workers, and other care team members to coordinate care.
Document all visits, assessments, and progress notes accurately in compliance with CMS/OASIS requirements.
Qualifications
Current State License as a Physical Therapist or Occupational Therapist.
Home health experience preferred but not required.
Knowledge of Medicare/Medicaid and home health documentation requirements.
Excellent interpersonal and communication skills.
Ability to work independently, manage time effectively, and travel to patient homes.
CPR/BLS certification.
