As a Hospice Aide/Homemaker, you'll provide compassionate, hands-on care that enhances comfort, dignity, and quality of life for terminally ill patients in their homes and care facilities. This rewarding position is perfect for a caring professional who finds purpose in providing personal care with empathy and respect. You'll be a valued member of our interdisciplinary team, working under the guidance of registered nurses to deliver individualized care that truly matters.In this role you will be responsible for:Follow the individualized Hospice Care Plan for each patient while adapting to assignment changes and varying care environmentsAssist patients with activities of daily living including bathing, dressing, grooming (skin/hair/nail), toileting, and oral hygieneAssist with meal preparation and help patients with eating and fluid intake to ensure patients receive adequate nutritionMonitor and record vital signs including temperature, pulse, respiration, and blood pressureHelp with transfers, ambulation, and positioning to ensure patient comfort and safety using proper body mechanics and assistive devices such as Hoyer lifts, wheelchairs, and gait beltsFollow fall precautions and respond appropriately to emergency situationsMaintain a safe and clean environment in accordance with care standardsDocument services provided and pertinent findings during each visitObserve and report changes in patient condition to the supervising RN within 24 hoursVerify assignments, review care plans, and confirm visit times with patients and facilitiesCollaborate with RNs, Social Workers, Chaplains, and other team membersCommunicate clearly and professionally with patients, families, facility personnel, and team membersAttend meetings and participate in ongoing trainingRequired QualificationsMust hold current certification as a nurse aide and be in good standing on the state nurse aide registryMinimum 1 year of experience as a hospice aide, home health aide, or certified nursing assistant preferredAbility to read, write, and follow written and verbal instructions in EnglishValid driver's license and reliable transportation with valid automobile insuranceWillingness to travel frequently within the assigned service areaEssential Skills and QualitiesUnderstanding of and commitment to the hospice philosophy of careComfortable providing specialized care to terminally ill patientsEmpathy and compassion for individuals who are ill, frail, or impairedSelf-directed with ability to work independently with minimal supervisionCalm, empathetic demeanor in emotionally sensitive situationsFlexibility and adaptability to handle schedule changes and varying patient needsStrong communication and listening skillsRespectful approach to patient dignity and privacyLocation Office Location: Jackson, MIMain Service Area: Jackson and surroundingThis position visits facilities and homesHoursMonday to Friday, 8:00 AM to 5:00 PM, with weekend rotationPay Range$18—$22 USDBackground ScreeningOptimal Care conducts a background screening upon acceptance of a contingent job offer.
