Center Manager
PACE, a member of Empath Health is seeking a detail-oriented Center Manager to oversee one of our Empath Life PACE location in Bradenton, FL that provides comprehensive medical and social services allowing seniors to remain safe and independent in their community. As part of our leadership team, you will make a direct impact on the lives of older adults and their families while helping shape the future of integrated senior care. The Center Manager oversees and manages the daily center operations of the Bradenton program. What is PACE?The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain independent and safe in their own homes. • Relationship-Based Care with Real Impact: Build lasting connections with participants, supporting their independence and mobility—this is a meaningful alternative to inpatient rehab or traditional outpatient care. Pace is an insurance program for individuals 55 and older and that meet certain criteria. PACE is focused on maintaining and supporting individuals as well as caregivers, facilitating them to remain living in the community for as long as possible. PACE has a medical clinic, and adult day center and can also provide care in the home. The PACE interdisciplinary team is comprised of a PCP, nurse, social worker, physical therapist, occupational therapist, registered dietician, activity coordinator/recreational therapist as well as other disciplines. What You'll DoAs a PACE Center Manager, you will oversee daily center operations ensuring a high standard of care and compliance with all regulations. Reporting to the Executive Director, you will: Oversee daily operations of the Bradenton PACE Center, ensuring high-quality, person-centered care and full regulatory compliance. Lead, mentor, and support day center staff, CNAs, and assigned team members to maintain a collaborative, high-performing environment. Coordinate interdisciplinary care planning, participant services, and family meetings to support safe community-based living. Manage hiring, onboarding, scheduling, evaluations, and performance improvement for center staff. Partner with leadership to monitor budgets, operational standards, and quality outcomes. Serve as a key liaison among PACE disciplines, internal departments, external providers, and participant families to ensure seamless communication and service delivery. Florida professional license (RN, Social Work, Therapy, etc.) preferred. Knowledge of the physical, mental, and social needs of older adults. Strong leadership, problem-solving, and team-building skills. Successful completion of competency assessment upon hire and annually thereafter. Valid Florida driver's license, proof of insurance, and reliable transportation. Preferred QualificationsExperience in quality/compliance or managed care/capitated programs. Professional background in Nursing, Social Work, Therapy, Dietetics, Gerontology, or related field. Demonstrated ability to manage multidisciplinary teams and drive quality outcomes.
