Clinical Operations Resource Nurse - Triage

Description: Current opening: Full Time position (32-40 hours per week). This position will be a hybrid role with opportunity to work from home upon completion of orientation. Reporting to the Senior Director of Hospice, the Clinical Operations Resource Nurse is responsible to provide support for home care and hospice patients through telephone triage. This position is instrumental in assessing and prioritizing patient care needs. This role is essential in providing interdepartmental collaboration and support. The Clinical Operations Resource Nurse works collaboratively with home care and hospice leadership and clinical teams. Requirements: Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC) Registered Nurse or Licensed Practical Nurse One-two years of home health or hospice experience One-two years of telephone triage experience preferred
