The hospice nurse evaluates clients' physical, emotional, psychosocial, and spiritual needs and develops appropriate care plans and interventions accordingly. The nurse is the case manager and oversees care to ensure the highest quality of care is maintained.Essential Responsibilities:Coordinates the care of the client and services, which may include the social worker, the chaplain, an in-home aide, the physician, and/or a volunteerAssesses the client and family within 24 hours of admission to HospiceProvides skilled nursing care as determined by the IDG plan of care.Provides education to clients, families, and/or others in care techniques specific to the clientServes as client/family advocate by maintaining regular communication with physicians and any other resources when neededParticipates in providing bereavement support to clients and familiesDocuments and ensures appropriate use of medications, DME, and suppliesParticipates in weekly meetings and IDG meetingsProvides an appropriate care plan and leaves in the home for other staff members and contract staff personnelMonitors the recertification/reauthorization processAccurately documents and follows through with the revocation processReports regularly to the Hospice Administrator, client updates, and reports statusMaintains confidentiality in all aspects of the jobPromotes personal safety and a safe environment for clients and co-workersMay complete In-Home Aide annual performance appraisals and assist with yearly competency testingParticipates in on-call rotationParticipates in performance improvement initiatives as requestedMay participate in the orientation of new staffMay act as a preceptor for new staffRepresents the nursing profession in a positive mannerMay participate in community educational activities as neededPromotes and participates in professional development for nursing and palliative carePerforms additional duties as requestedOn-call rotationCovering counties to include: Bladen, Cumberland, Harnett, Hoke, Lee, Robeson, Sampson, Scotland, and surrounding counties.Requirements:Qualifications:Active NC nursing licenseFunction as a core member of the IDGAbility to speak in front of groupsMaintain a high-quality customer service approachADN requiredAt least one year of hospice or home health nursing experience preferredPossess active listening and excellent assessment skillsWorking knowledge of a variety of computer programs is desiredPhysical Demands:Ability to sit for long periods, either while driving, participating in IDG meetings, visiting clients, or performing computer work. xevrcyc This position may require frequent bending, reaching, lifting, talking, and listening.Compensation details: 75 Yearly SalaryPI7976dc09d2a
