Job DescriptionSuccess Coach / Social WorkerNew Hanover and Multiple Counties in Southeastern North CarolinaPosition SummaryThe Success Coach / Social Worker provides a voluntary, trauma-informed case management, advocacy, coaching, and support services to youth, young adults, and families who have been reunified with their children or who have recently adopted children from the foster care system across New Hanover County and surrounding counties in Southeastern North Carolina. This position focuses on helping clients achieve stability, independence, educational success, employment readiness, and healthy social-emotional development through individualized support plans and community-based services.The Success Coach / Social Worker works collaboratively with schools, community agencies, healthcare providers, employers, courts, and family systems to ensure clients have access to resources and opportunities that promote long-term success.Key ResponsibilitiesClient Support & Case ManagementProvide individualized coaching, mentoring, and social work support to assigned clients.Conduct assessments to identify client strengths, needs, barriers, and goals.Develop and implement individualized success plans focused on education, employment, housing, life skills, mental health, and overall well-being.Maintain regular contact with clients through in-person meetings, phone calls, virtual check-ins, home visits, school visits, and community outreach.Monitor client progress and adjust service plans as needed.Assist clients with crisis intervention, conflict resolution, and problem-solving strategies.Support youth and families navigating transitions, including school changes, independent living, employment, or community reintegration.Advocacy & Resource CoordinationConnect clients and families with community resources, including healthcare, behavioral health, housing, transportation, education, workforce development, and financial assistance programs.Advocate for clients within school systems, court systems, social service agencies, and healthcare environments.Build and maintain collaborative relationships with community partners throughout Southeastern North Carolina.Coordinate referrals and follow-up services to ensure continuity of care.Education & Employment SupportSupport academic achievement, school attendance, graduation planning, and post-secondary goals.Assist clients with career exploration, resume development, interview preparation, and job readiness skills.Encourage development of independent living and self-advocacy skills.Facilitate workshops or group sessions related to life skills, emotional wellness, budgeting, employment readiness, and healthy relationships.Documentation & ComplianceMaintain accurate, timely, and confidential client records in accordance with agency policies and state/federal regulations.Complete case notes, assessments, service plans, reports, and documentation within required timelines.Track outcomes, participation, and progress toward established goals.Participate in team meetings, supervision, training, and professional development activities.Community EngagementRepresent the organization at community meetings, networking events, schools, and partner agencies.Assist with outreach efforts and recruitment of program participants.Participate in collaborative initiatives that support youth and families in the region.Geographic CoverageThis position serves New Hanover County and multiple counties throughout Southeastern North Carolina. Frequent travel within the service area is required.QualificationsRequired QualificationsBachelor’s degree in Social Work, Human Services, Psychology, Counseling, Sociology, Education, or a related field.Minimum of 1–3 years of experience working with youth, families, or vulnerable populations.Strong case management, communication, and interpersonal skills.Ability to work independently and manage a flexible schedule.Valid driver’s license, reliable transportation, and ability to travel throughout the region.Proficiency in Microsoft Office and electronic documentation systems.Preferred QualificationsNorth Carolina Social Work licensure (LCSWA, LCSW) or related credential.Experience with trauma-informed care, youth development, foster care, juvenile justice, behavioral health, or family services.Knowledge of community resources within Southeastern North Carolina.Bilingual skills are a plus.Knowledge, Skills, & AbilitiesUnderstanding of trauma-informed practices and culturally responsive care.Ability to build trust and positive relationships with diverse populations.Strong organizational and time-management skills.Ability to maintain confidentiality and professional boundaries.Effective crisis management and conflict resolution skills.Strong advocacy and problem-solving abilities.Ability to collaborate effectively with multidisciplinary teams and community partners.Work EnvironmentCombination of office, school, community, and home-based environments.Regular local travel required.Occasional evening or weekend hours may be required to meet client needs.Physical RequirementsAbility to travel frequently throughout the assigned region.Ability to sit, stand, walk, and use standard office equipment.Ability to lift and carry materials up to 25 pounds occasionally.Compensation & BenefitsCompetitive salary based on education and experience.Mileage reimbursement for approved travel.Health, dental, and vision insurance.Paid time off and holidays.Retirement plan options.Professional development and training opportunities.Equal Opportunity StatementThe organization is an Equal Opportunity Employer and values diversity at all levels of the workforce.
