Wraparound Service Coordinator - DCFS

DescriptionPOSITION SUMMARYThe Wraparound Service Coordinator works in collaboration with the youth, family, guardian, caseworker and DCFS. Responsible for the design and delivery of services that meet the youth's needs and provide support to youth and family who have been referred to the Wraparound Program.ESSENTIAL FUNCTIONS AND RESPONSIBILITIESEvaluate and interpret the referral information. Complete a strength based Discovery Assessment with the child and family and the Child and Adolescent Needs and Strength Assessment (CANS) at time of admission and discharge.Development, document, and maintain a Crisis Plan and individualized Wraparound Plan in conjunction with the youth, parent, guardian, caregiver and caseworker of referring agency.Conduct weekly face to face home/community visits with the assigned youth and their families/support system. Participate in monthly Child and Family Team Meetings for each assigned youth to review care plan and progress on needs. Maintain meeting minutes for CFTM as needed.Using resources and available flex funding to assure that services are based specifically on the needs of the child and family and are authorized by the Wraparound Plan and are reimbursed in a timely manner.Deliver strength-based, family centered, culturally competent and trauma informed facilitation services.Engage with treatment providers, school and other collateral contacts to ensure youth's needs are being met.Oversee transition to any ongoing care after discharge from Wraparound Program.Maintains case records and notes in a timely and professional manner; in accordance with the Wraparound Program Contract.Completes other duties as assigned.RequirementsQUALIFICATION REQUIREMENTSStrong organizational and time management skills.Excellent interpersonal and written communication skills.Must possess the knowledge, personal qualities and practice skills to work effectively with professionals in other organizations and from other disciplines and to provide services in a manner that reflects understanding and appreciation of multiculturalism.The ability and willingness to actively participate in a fast-paced, multidisciplinary team environment.Knowledge of community programs, services and evidence-based practices and a variety of treatment modalities used to work with youth and their family to navigate child serving systems.Must meet DCFS/CANTS Clearance RequirementsMust be at least 21 years of ageMust possess and maintain a valid driver's license and maintain proof of automobile insuranceEDUCATION AND/OR EXPERIENCE* Bachelor's Degree in a counseling and guidance, rehabilitation counseling, social work, education, psychology, or related human service field along with three years' supervised experiences.CARE & TCIHoyleton has adopted the CARE (Children and Residential Experience), developed by Cornell University, to guide our culture and practice.
