The LPN will work collaboratively with interdisciplinary staff to deliver safe, compassionate, and patient-centered care while maintaining clinical excellence in a fast-paced SNF environment. Key Responsibilities Provide direct nursing care to SNF residents under the direction of the RN and provider team Administer medications and treatments according to physician orders and facility protocols Monitor resident conditions and report changes promptly Perform routine nursing assessments and document care accurately Support admissions, transfers, and discharge processes Maintain infection prevention and patient safety standards Coordinate with rehabilitation, therapy, and interdisciplinary teams Educate residents and families regarding care plans and treatments Ensure compliance with facility and regulatory documentation standards Required Qualifications Active New York State LPN License Current BLS certification Minimum 2 years of LPN experience (SNF or comparable setting preferred) Strong clinical assessment and communication skills Ability to work independently and prioritize resident care needs Preferred Experience Skilled Nursing Facility (SNF) experience Long-term care or rehabilitation nursing background Experience managing multiple patient needs in a structured care environment Work Location: In person About Hanker Systems Inc Healthcare staffing at Hanker Systems Inc is focused on delivering the right talent to support quality patient care.
