Social Worker (LCSW/LICSW)

Position Overview:Facility seeks an experienced Social Worker (LCSW or LICSW) to join our Care Coordination team on a long-term assignment. The Social Worker will play a critical role in providing psychosocial support, case management, and care coordination services to patients and their families, ensuring high-quality outcomes and smooth transitions of care.Key Responsibilities:Assess patients' psychosocial needs and collaborate with healthcare teams to develop individualized care plans.Provide counseling and emotional support to patients and families coping with illness, hospitalization, or life changes.Facilitate discharge planning, including coordination with community resources, home health agencies, and long-term care facilities.Advocate for patients' rights and access to resources.Conduct thorough assessments and maintain accurate documentation in compliance with hospital policies and regulations.Serve as a liaison between the patient, family, and healthcare team to promote effective communication and decision-making.Participate in interdisciplinary team meetings and case conferences.Qualifications:Active licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in the state of Massachusetts.Minimum of 2 years of clinical social work experience in a hospital or healthcare setting (no new graduates).Strong knowledge of community resources, care coordination processes, and discharge planning.Exceptional interpersonal, organizational, and communication skills.Ability to work both independently and as part of a multidisciplinary team.
