The schedule for this role is Monday - Friday 7 :00 am to 4:00 pm.This is a full-time position that will demonstrate competency in delivery of quality patient care to patients ranging in age from children to geriatrics. The LPN is responsible for implementing the plan of care under the direction of an RN and in accordance with the patients’ express wishes and in full compliance with the Nurse Practice Act and agency policy, procedure and standards of care.Scroll down to find the complete details of the job offer, including experience required and associated duties and tasks.ResponsibilitiesAssess physical environment/safety needs of patient.Provide in-patient facility nursing care under the direction of an RN and according to the patient’s plan of care.Collaborate with patients, significant others, and interdisciplinary group (IDG) members to focus on holistic needs of patient in order to assure satisfactory outcomes for patients.Act in a professional manner to collaborate with physicians, other health care providers, and IDG members and make timely reports of pertinent information to appropriate health care professionals.Complete accurate and timely documentation of nursing assessments, interventions, and updates to plan of care according to the nursing standards of this agency.Administer and document the administration of medications according to physician’s orders.Accurately account for narcotics at the beginning and end of each shift.Partner with certified nurse aides to perform personal care for patients as directed by supervisor.Have a thorough understanding of the facility’s policies and procedures for emergency situations and be able to perform assigned tasks during practice drills to demonstrate this knowledge.Provide patient and his/her significant others with appropriate education to support safe care of the patient in the Hospice Home and in accordance with the current plan of care.Participate in own professional development by maintaining required nursing competencies and attending educational offerings.Monitors and manages the appropriate use of patients’ medical equipment and suppliesParticipate in the identification of clinical or operational performance improvement opportunities and assists in performance improvement activities.Actively participates in the orientation and training of new employees and visiting students of all disciplines.Maintain patient confidentiality at all times.Performs other duties as assigned by supervisor.ExpectationsModel professionalism and empathy by consistently demonstrating compassion, respect, teamwork, and dedication in all interactions with coworkers, customers, and partners.Adhere to all TL policies, procedures, and organizational guidelines, ensuring compliance in daily work and decision‑making.Deliver care and services in accordance with ACHC Standards and Medicare Conditions of Participation when applicable to your role.Comply with all relevant state and federal laws, regulations, and professional standards within your defined scope of responsibility.Promptly communicate concerns, issues, or potential risks to your immediate supervisor or another member of the management team to support a safe and effective work environmentRequirementsLicensed Practical Nurse with current NC license.Current CPR certification.Minimum 2 years recent clinical experience. Hospice and/or skilled nursing home, hospital or healthcare facility experience preferred.Able to seek out resources and work collaboratively under the direction of an RNAble to communicate clearly with patients, families, visitors, IDG team, physicians, facility staff, leadership and othersAble to teach patients and families in accordance with the nursing plan of careKnowledgeable about reimbursement sources, Medicare regulations, licensing laws, and accreditation standards.Competent computer skills.Must have reliable transportation.Must be able to work on a shift basis.May be required to lift and/or carry items up to 50 pounds.Must have sensory abilities to complete physical assessment, communicate with patient/family care providers and physician and to complete the reporting and follow-up documentation responsibilities of the job.Environmental FactorsTasks with potential for exposure to blood borne pathogens.May be potentially exposed to communicable diseases or bodily fluids. xaygatp Subject to varying and unpredictable situations in the Hospice Home Environment.
