DescriptionPosition: Bilingual Continuous Care Coach - (Remote - Work From Home COA)Job Type: Non-exempt - Full-timeSchedule: Monday to FridayCandidates must be bilingual, fluent in speaking both English and Spanish.Pay: $21-24.50 (Eligible for bilingual differential with successful completion of Spanish language assessments)Location: RemoteSummaryPosition Overview: As a Bilingual Continuous Care Coach, you will be responsible for assessing, coordinating, and managing the care needs of patients with chronic eye diseases. You will work closely with patients, their families, healthcare providers, and our internal teams to develop and implement personalized care plans that address their specific requirements. Your primary goal will be to ensure that patients receive high-quality care and support throughout their journey.Top candidates will have the ability to enthusiastically interact with up to 15 patients per day to provide encouragement, empathy, and support. This includes meeting established goals, metrics, and expectations outlined for patient caseload management.Essential FunctionsManage the patient caseload and communicate as frequently as necessary, based on assessing the patient needCommunication and engagement with patient, family members, guardian or caretaker, surrogate decision makers, and/or other professionals regarding aspects of careCommunication with home health agencies and other community services utilized by the patientCollection of health outcomes data and registry documentationPatient and/or family/caregiver education to support self-management, independent living, and activities of daily livingAssessment and support for treatment regimen adherence and medication managementIdentification of available community and health resourcesFacilitating access to care and services needed by the patient and/or familyOngoing review of patient status, including review of laboratory and other studies not reported as part of an E/M serviceDevelopment, communication, and maintenance of a comprehensive or disease-specific care planUse motivational interviewing to assess and address any barriers their patients may be experiencing and work in a holistic manner to help resolve these barriersMust be able to work flexibility within it but we do require you work during normal business hours in your patients’ time zoneMust have the ability to interact with up to 15 patients per day, providing encouragement, empathy, and support. This includes meeting established goals, metrics, and expectations outlined for patient caseload management.Communicate regularly with other Team Leads, Managers, Directors, and appropriate stakeholders as needed to as necessary to promote healthy exchanges of crucial information Perform additional responsibilities as required to support Lumata Health’s goals and objectives RequirementsRequired SkillsAbility to work in a team environment with very little supervisionOutstanding communication skills, both written and verbalExcellent organizational skillsExcellent customer service, interpersonal and communication skillsExcellent time management skills with the ability to meet deadlinesStrong consultative skills, ability to provide guidance to patientsAbility to provide feedback and insights to contribute to patientsAbility to work across various time zones, effectively work remote with reliable WIFI allowing for consistent conferencing/calls.Ability to consistently phone conference as the position requires and when needed.Ability to speak fluently in both English and Spanish.Education And Experience2+ years of work experience in eye careCertified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) requiredProficient with Electronic Medical Record (EMR) systems, preferredPhysical RequirementsMust have a home office that is secure and free of distractions (HIPAA-compliant workspace)Prolonged periods of sitting at the desk and working on a computerMust be able to see, hear, speak, type, call and virtually meetMust be able to communicate both verbally and in writingWork EnvironmentIn order to perform the essential functions of this job, you must have a dedicated remote workspace, and a wireless connection with a minimum of 50 Mbps of download speed and 10 Mbps of upload speed of dedicated internet bandwidth for each person working from home; the workspace is private and has low to no noise level as you will be on phone calls with patients. It is very important that your workspace is HIPAA compliant; your company issued laptop/pc must maintain visibility that only you can access as you'll be handling patient data.
