Join us and be part of something bigger – helping to simplify health care one person, one family and one community at a time.The Virtual Primary Care Nurse Practitioner works in collaboration with a dedicated care team to provide evidence-based, primary care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients 18 years of age and older. This full-time clinical position is 100% remote. The care team will work with NP daily to drive better outcomes for the entire panel. Patient-Centered Quality and SafetyDemonstrate knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional care patientsEvaluate and treat patients' medical needs with safe and high-quality careDocument all patient care within an electronic health record in line with current policies and proceduresWork as a member of the care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is provided for all patients and at all timeProvide patient centered care that is compassionate, appropriate and effective for the promotion of health, prevention of illness and treatment of diseaseReferral to appropriate Specialists and patient care components as appropriatePossess an awareness and a working understanding of quality measurement in the management of chronic disease conditionsDocument patients that have expressed a desire to join care team panel, using approved EMR workflowsEpic Optimization, Care Pathways, etc.)Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing~ 5+ years of recent Primary Care experience as a Nurse Practitioner in family medicine or primary care~ Ability to work remotely in a care team culture~ Take after-hours and/or overnight call to support patient careVirtual Care Team members must meet minimum requirements for remote care delivery, including broadband connectivity, a quiet setting with a neutral background to conduct visits from, and the ability to uphold patient privacy per CVSH guidelinesWhile performing the duties of the job, the employee is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responsesSpecific vision abilities include the ability to view and read a computer screen and other electronic devicesCompletion of master's degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role requiredFull timeThis pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
