Case Manager In-Home Post-Crisis Stabilization Services for Youth & Families Organization: Collaborative Trauma Solutions Position Type: Full-Time Compensation: Competitive salary based on experience plus mileage reimbursement Location: Candidates must reside in and be willing to travel throughout Mesa County, Colorado. Expected Start Date: Immediately Make a Difference Where It Matters Most Collaborative Trauma Solutions is seeking a compassionate, organized, and motivated Case Manager to join our growing Community-Based Services team. This position provides intensive, trauma-informed, in-home post-crisis stabilization services to youth and families following a behavioral health crisis. Our goal is to help families remain safely together, strengthen protective factors, reduce future crises, and connect families with the services and supports they need to thrive. What You'll Do As a Case Manager, you will: Provide strengths-based, trauma-informed case management services to youth and families following a behavioral health crisis. Conduct home and community visits to assess needs, identify strengths, and support stabilization. Develop individualized service plans with youth and caregivers that promote long-term success. Coordinate care among therapists, schools, hospitals, medical providers, child welfare, probation, and other community partners. Help families navigate behavioral health, educational, medical, and social service systems. Connect families with community resources and natural supports that improve long-term stability. Support caregivers in understanding treatment recommendations and accessing appropriate services. Advocate for youth and families while promoting self-advocacy and independence. Participate in multidisciplinary team meetings and collaborate closely with Community- Based Services staff. Complete all required documentation accurately and within agency timelines. Travel throughout Mesa County to provide services in homes, schools, and community settings. We're Looking for Someone Who Enjoys working with children, adolescents, and families. Is compassionate, dependable, and committed to helping families succeed. Has excellent communication, organizational, and problem-solving skills. Can work independently while collaborating effectively within a multidisciplinary team. Understands or is willing to learn trauma-informed and family-centered practices. Is comfortable providing services in homes, schools, and community settings. Has reliable transportation, a valid driver's license, and proof of automobile insurance. Is willing to travel throughout Mesa County (mileage reimbursed). Required Qualifications Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, or another related behavioral health field. Experience in case management, child welfare, behavioral health, education, juvenile justice, or community-based services. Preferred Qualifications Experience working with youth, families, or individuals experiencing behavioral health challenges. Knowledge of trauma-informed care, family systems, and recovery-oriented practices. Experience providing post-crisis stabilization or intensive community-based services.
