DescriptionJOB TITLE: Certified Nursing Assistant – Supported LivingDEPARTMENT: Community LivingSUPERVISOR'S TITLE: Program Manager(s) – Supported Living and Assistant Director- Supported LivingPOSITIONS SUPERVISED: NoneJOB STATUS: Full-time, Non-ExemptSCHEDULE: 8 hour or 12 hour shifts availableSummaryProvide direct, person-centered care and support to individuals in Supported Living settings. This role focuses on assisting with personal care, promoting independence, and supporting meaningful participation in home and community life while ensuring health, safety, and compliance with regulatory standards.Key Responsibilities Deliver services in accordance with each individual’s Individualized Support Plan (ISP) Assist with implementation and monitoring of ISP goals and objectives Provide personal care and health-related support (e.g., bathing, toileting, feeding) Support individuals with daily living skills and household routines Facilitate community involvement, recreation, and social engagement Provide or coordinate transportation to appointments, employment, and activities Monitor individual progress and communicate updates to the team Promote self-advocacy, independence, and informed decision-making Build and maintain positive relationships with individuals, families, and service providers Provide crisis intervention and emergency assistance as needed Ensure the safety and well-being of individuals served Follow and implement structured activities that promote independenceQualifications & Skills Active Indiana Certified Nursing Assistant (CNA) certification Strong communication and interpersonal skills Ability to demonstrate empathy, patience, and professionalism Ability to work independently and as part of a team Understanding of person-centered care and advocacy Ability to support individuals with diverse needs and abilities Strong attention to detail, especially in documentation and care tasksPhysical & Work Requirements Ability to work a flexible schedule, including evenings, weekends, and holidays Ability to work every other weekend Ability to provide personal care with dignity and respect Ability to assist with transfers and mobility support Ability to lift and carry up to 40 lbs frequently Frequent standing, walking, bending, and reaching Ability to respond effectively in emergency situations Valid driver’s license, reliable transportation, and proof of insuranceCertifications & Training Requirements CPR, First Aid, and Bloodborne Pathogens certification Certified Restraint Training Core A & B certification Completion of required agency and regulatory trainings (e.g., Human Rights, Safety, Medication Administration)Additional InformationFor more information, please reach out to Brian James at bjames@rauchinc.org$350 Sign-on Bonus eligible position.
