Dynamic & Fl le: This role involes traveling within the community, providing direct, hands-on support and assisting the Designated Coordinator with key tasks. Crisis Preventio Support: Play an essential rle in crisis prevention, ensuring individuals receive the care and support they need, especially during emergencies. Team Collaboration: closely with the Dsignated Coordinator and care team to ensure smooth service delivery and compliance with regulations. Candidates should be flexible and adaptable to potential changes in job responsibilities. Key Responsibilities (Cur nt): Provide direct care and s t to individuals living independently, helping them acquire essential adaptive skills. Assist individuals in develop personalized 24-Hour Emergency Back-Up Plans. Support the Designated Coordina with clerical tasks, ensuring efficient service delivery. Document progress, report inciden and maintain compliance with 245D regulatory guidelines. Collaborate with a wide range of pr ssionals, including waivered service providers, mental health teams, and social workers. Participate in an on-call rotation fo 4-Hour Emergency Assistance services. Comprehensive Benefits Package: 401(k ith company match up to 3.5% (e e after 1 year). Hold at minimum a 2-year degree in Human Services or a related field, AND at minimum ears of direct experience working with individuals with Intellectual/Developmental Disabilities or related conditions Working Conditions: The Unit Based Services Program Assistant works in various communi settings, frequentl traveling to clients' homes.
