Site Manager – Tenant Case Management ProgramReports To: Clinical Professional Development DirectorLocation: Ashe, Caldwell, Haywood, and Wilkes Counties, On-SitePosition SummaryThe Site Manager is responsible for providing non-clinical, person-centered support services toindividuals receiving Tenant Case Management through Covenant Case Management Services.This position assists members in maintaining housing stability, accessing behavioral health andcommunity services, building life skills, and supporting whole health and resiliency goals inaccordance with the Vaya Health Tenant Case Management program requirements.Key ResponsibilitiesMember Support & Service DeliveryMaintain scheduled contact with assigned members to support housing stability,independent living skills, and crisis prevention.Assist members with developing and achieving whole health and resiliency goalsLink members to behavioral health, medical, community, and natural support resourcesas needed.Participate in the creation and updates of Person-Centered Plans, Crisis Plans, orHousing Stabilization Plans for members not linked to a behavioral health provider.Provide education, engagement, and support to members and their families/caregiversusing non-clinical approaches.Maintain a working knowledge of the Eviction Prevention Protocol and supportintervention steps as needed.Documentation & ReportingDocument all service contacts and interventions using required logs and monthly servicenotes in accordance with program standards.Assist clients with the completion of their annual recertification documentationSubmit accurate and timely documentation, signatures, and required reportsMaintain applicant/waitlist tracking as assigned and support Residential SelectionCommittee processes.Maintain confidentiality and comply with HIPAA and Medicaid regulationsAssist clients with required work order and maintenance formsCoordinate services with local vendorsCollaboration & CommunicationWork closely with Vaya Health Residential Housing Specialists, ARC of NC PropertyManagement staff, and internal Covenant Case Management team members.Participate in interviews for housing applicants as requestedNotify supervisors of member concerns, safety risks, or eviction risks promptlyRepresent the agency in a professional and ethical manner in community settingsQualificationsRequired:High School diploma or equivalentValid driver’s license, reliable transportation, and ability to travel to work siteAbility to pass background check and required credentialingStrong communication, documentation, and interpersonal skillsPreferred:Experience working with MH/SU/IDD/TBI populationsExperience in case management, supportive housing, or human servicesAssociate or Bachelor’s degree in Human Services, Psychology, Social Work, or relatedfieldWork Environment & Physical RequirementsField-based position requiring travel to housing sitesAbility to sit, stand, walk, and lift up to 25 lbs occasionallyMust maintain professional boundaries and safety practicesEqual Employment Opportunity StatementCovenant Case Management Services actively encourages applications from individuals of all backgrounds and lived experiences.
