Job Summary
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive primary care services to patients of all ages, focusing on health promotion, disease prevention, and management of chronic conditions. The FNP will work collaboratively within the PATIENTS HOME environment, ensuring high-quality patient care and fostering long-term relationships with patients and their families.
Duties
Conduct thorough assessments and evaluations of patients to determine their health status in home.
Develop and implement individualized care plans based on patient needs and preferences.
Provide primary care services including diagnosis, treatment, and management of acute and chronic illnesses.
Offer specialized care in pediatrics and dementia care, ensuring appropriate interventions for diverse patient populations.
Utilize Athena health software for electronic health records management to maintain accurate patient documentation.
Employ aseptic techniques in all clinical procedures to ensure patient safety.
Educate patients and families about health maintenance, disease prevention, and treatment options.
Collaborate with other healthcare professionals to coordinate comprehensive patient care.
Experience
A Master’s degree in Nursing with a focus on Family Nurse Practitioner training is required.
Previous experience in primary care settings is preferred, particularly in pediatrics or geriatric populations.
Proficiency in using electronic health record systems such as Athena health is highly desirable.
Demonstrated ability to provide compassionate patient care while maintaining professionalism in a clinical environment.
Strong communication skills to effectively interact with patients, families, and team members.
