The ideal candidate will have experience or a strong interest in Survivorship care and will work collaboratively under the supervision of multiple Oncologists and other healthcare professionals to deliver high-quality, patient-centered care.Key ResponsibilitiesConduct comprehensive survivorship assessments including history, physical examinations, and interpretation of diagnostic studies.Develop, implement and regularly update individualized survivorship care plans following completion of cancer treatment.Summarize treatment history (including surgery, chemotherapy, radiation, and immunotherapy) and outline follow-up careCoordinate transition from active oncology care to long-term survivorship.Monitor for cancer recurrence and secondary malignanciesOder and interpret appropriate surveillance labs, imaging, and diagnostic tests.Identify and manage late and long-term effects of cancer and its treatment, including referral to rehabilitation, specialty services, or supportive therapies as needed.Screen patients for psychosocial or emotional impacts and provide counseling and referrals as needed.Counsel survivors on health promotion, disease prevention, and age/risk appropriate screenings and vaccinationsProvide patient and caregiver education.Maintain compliance with organizational policies, regulatory guidelines, and best practice standards.QualificationsGraduate of an accredited Physician Assistant or Nurse Practitioner ProgramNational Advanced Practice Registered Nurse Certification (APRN) OR national board certification as a Physician Assistant.Current Physician Assistant or APRN license in the State of Florida.AOCNP CertificationBLS/ACLSPreferred Skills2 years of Oncology ExperienceExperience with Epic Electronic Medical RecordCompensation And BenefitsCompetitive salary based upon experienceHealth, Dental, and Vision Insurance403(b) with employer matchPaid time off and CME allowanceOnboarding and transition to practice supportMalpractice insurance coverageSupportive team environment with opportunities for professional growth and development.Our Commitment Rooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings.
