Delivers assigned, delegated, direct care consistent with the individualized treatment plan. Performs routine care consistent with established standards policies and procedures. Maintains a safe and therapeutic treatment environment.Major Duties and Responsibilities:Collects and reports health status dataGathers objective data & observations about the patientDifferentiates between normal & abnormal observationsReports changes in patient status or abnormal findings to RNParticipates as a member of the interdisciplinary team Implements established plan of acre for each patientUnderstands the plan of care for each assigned patientCompletes interventions identified on the plan of care Delivers delegated aspects of direct careProvides care based on the established treatment planInvolves patient and parent/guardian in the care processLeads Dialectical Behavioral Therapy (DBT), Radically Open Dialectical Behavioral Therapy (RO-DBT) and Wellness Groups in a classroom setting.Engages in individual & purposeful interactions with patients and parent/guardianCompletes vital signs, and weights as directed Monitors patient and environmental safety continuouslyMaintains a calm, healing environmentSecures patient’s belongings Documents in the electronic medical record (EMR)Completes group notes on assigned groupsReflects individual patient’s progress towards treatment goalsArticulates interventions utilized and responses to the interventionsNotes are factual and conciseObtains RN co-signature for EMR notes Demonstrates understanding of current therapeutic techniquesContinuously participates in learning opportunities to expand inventory of intervention optionsApplies learning and experience to patient care Effectively responds to escalating behaviorObserves patients for risk of aggressive behaviorDemonstrates an ability to identify early changes in behavior that indicate escalationEngages with patients who are exhibiting early behavioral changesApplies alternatives to physical intervention Facilitates customer satisfactionIdentifies issues of customer serviceActs to resolve customer service issues within available options in collaboration with team membersRefers complex and unresolved issues to the appropriate partiesParticipates in meetings with referral sources and external agencies Provides information to patients & familiesPresents information to patients and parent/guardian utilizing age specific principlesReviews comprehension of information Participates in the continuous improvement of the organization through the quality improvementprocessCompletes audits for assigned indicatorsReports any actual or potential situation that impacts patient safetyImplements actions that result in continuous improvement of quality & complianceParticipates in the organization wide quality measures as assigned Initiates emergency action when indicatedIdentifies emergency situations occurring in the programResponds to the emergency situation with immediate attention in a controlled & skilled manner Perform other duties as assignedPosition Qualifications:BLS/PALS certification is required. May be obtained after hired.Bachelors’ Degree in nursing, psychology, social work or other related field with six (6) months of direct patient care in a mental health setting required. – OR –Associates Degree in nursing, psychology, social work or other related field with two (2) years of direct patient care in a mental health experience required. – OR – High School Diploma and GED with four (4) years’ experience with direct patient care in mental health setting required.Demonstrated ability to effectively facilitate groups.Demonstrates the knowledge and skills necessary to provide age-specific and developmentally appropriate care for the spectrum of populations admitted to LCOH.Ability to enthusiastically follow and model the Lindner Center of HOPE mission, vision and values.Physical Requirements:The physical demands of this position are consistent with those performed in a normal office environment, including occasional lifting, carrying, pushing or pulling, and sufficient manual dexterity to operate standard office equipment and ability to use a computer.
