Job Description Job Summary:A Homecare Physical Therapist provides skilled physical therapy services to patients in their homes, helping them recover from injuries, surgeries, or chronic conditions while promoting independence and improving overall mobility. This role focuses on personalized, patient-centered care in a home environment, requiring strong clinical judgment, adaptability, and communication skills.Key Responsibilities:Conduct comprehensive in-home evaluations of patients' physical abilities, functional limitations, and living environmentsDevelop and implement individualized treatment plans tailored to patient goals and home settingsProvide therapeutic exercises, manual therapy, gait training, and balance activitiesEducate patients and caregivers on safe mobility techniques, fall prevention, and home exercise programsMonitor patient progress and adjust treatment plans as neededDocument patient care accurately and in compliance with healthcare regulations and agency policiesCollaborate with physicians, nurses, occupational therapists, and other healthcare professionalsEnsure a safe and supportive home environment by recommending adaptive equipment or modificationsTravel to patient homes within assigned service areasQualifications:Doctor of Physical Therapy (DPT) or equivalent degree from an accredited programCurrent state licensure as a Physical TherapistValid driver's license and reliable transportationCPR certification (or willingness to obtain)Prior experience in home health or outpatient care preferredSkills & Competencies:Strong clinical reasoning and problem-solving abilitiesExcellent communication and interpersonal skillsAbility to work independently and manage time effectivelyAdaptability to varying home environments and patient needsCompassionate, patient-focused approach to careWork Environment:Patient homes in diverse settingsRegular travel requiredFlexible scheduling, including potential evenings or weekendsPhysical Requirements:Ability to lift, transfer, and assist patients with mobilityProlonged standing, walking, and bendingAbility to navigate different home environments safelyBenefits (may vary by employer):Competitive salary and mileage reimbursementHealth, dental, and vision insuranceContinuing education opportunitiesFlexible schedulingRetirement plan optionsObjective of the Role:To enhance patients' quality of life by improving mobility, reducing pain, and enabling safe, independent functioning within their home environment.
