Licensed Practical Nurse
Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including long-term care, assisted living, hospitals, and outpatient clinics. The LPN will utilize their expertise in medical procedures, patient assessment, and documentation to support patient recovery and well-being. Experience with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks is preferred. This role requires a dedicated professional capable of managing complex medical needs, including IV infusion, wound care, medication administration, and vital signs monitoring. The position offers opportunities to work with diverse patient populations, including seniors, individuals with developmental disabilities, and those requiring specialized care such as dementia or Alzheimer's.
Responsibilities
Conduct comprehensive patient assessments including vital signs, physical examinations, and health histories
Administer medications and injections accurately following physician orders
Perform IV infusions, catheterizations, and sterile procedures with aseptic technique
Document all patient interactions thoroughly in EMR/EHR systems such as Epic or Cerner
Assist with diagnostic evaluations including specimen collection/processing and medical imaging coordination
Manage airway care, ventilator support, and tube feeding as needed for critical or acute care patients
Provide wound care, suturing assistance, and post-operative discharge planning
Support patients with chronic conditions such as dementia or Alzheimer's through specialized memory care protocols
Collaborate with interdisciplinary teams in settings like ICU, emergency room, or hospice to ensure optimal patient outcomes
Educate patients and families on health coaching topics including nutrition, medication management, and discharge instructions
Maintain infection control standards and adhere to HIPAA regulations to protect patient confidentiality
Experience
Proven experience in diverse healthcare environments such as hospital medicine, ICU (Level I & II Trauma Centers), emergency medicine, or post-acute care settings
Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks
Experience administering injections, phlebotomy, venipuncture, and catheter care
Knowledge of ICD-9/ICD-10 coding for accurate medical documentation and billing purposes
Familiarity with specialized areas such as geriatrics, pediatrics (including neonatal ventilators), behavioral health, or home & community care is highly desirable
Prior roles involving critical care experience like ICU or trauma centers are advantageous
Ability to perform physical examinations and diagnostic evaluations competently
Strong understanding of physiology and anatomy relevant to nursing assessments and interventions
