Responsibilities include:Goal-Based Client SupportWork with the Care Right There team to implement client-specific goals developed by nursing and care managementTrack progress and adjust approaches as goals are met, modified, or changed due to improvement or decline in conditionDocument goal progress clearly and consistentlyTherapeutic & Functional ActivitiesFacilitate daily cognitive stimulation activities such as memory games, orientation exercises, puzzles, reading, and conversation engagementLead functional activities to support independence (dressing practice, fine motor tasks, handwriting, sorting, etc.)Assist with physical therapyaligned exercises as directed by the RN or therapist (range of motion, mobility exercises, balance support, safe ambulation)Encourage meaningful, structured activities (music therapy, art, cooking, routine-based tasks, and personalized engagement)Daily Personal Care & ADLsAssist with bathing, grooming, dressing, toileting, feeding, and hygieneSupport safe mobility, transfers, and prescribed exercise programsMaintain safety through supervision, fall prevention, and behavioral redirectionDementia & Alzheimer's Specialized CareUse appropriate dementia-care techniques, including validation, redirection, calm communication, and routine-buildingManage and document behavioral changes, confusion, anxiety, wandering, or agitationMaintain structured daily routines that promote stability and reduce cognitive stressTechnology Use & CommunicationUse caregiver technology tools (apps, tablets, AxisCare, communication tools) to:Log care notesTrack goal progressFollow digital care plansCommunicate with the office and nursing teamUse simple tech with clients when appropriate (music playlists, brain-game apps, memory prompts, digital photo albums)Household & Safety SupportPrepare nutritious meals and support hydrationPerform light housekeeping related to client care (laundry, dishes, room upkeep)Maintain a clean, organized, and therapeutic environmentAccompany clients to appointments when neededRequirements-Active NJ CHHA Certification (required) -Dementia/Alzheimer's training or certification (required) -Minimum 1 year of memory care experience -Ability to commit to live-in care schedules where you will reside at the patient's home 24/7, 7 days a week -Reliable transportation -Strong communication, patience, and problem-solving skillsCare Right There Home CareThere's no place like home Just as growing older can allow for a more relaxed lifestyle, it can also present new obstacles and challenges.
