Job Overview
We are seeking a dedicated Senior Care Coordinator to join our team and provide exceptional care coordination services to our senior clients. The ideal candidate will have a passion for caregiving and possess the necessary skills to support seniors in their daily activities.
Responsibilities
- Coordinate and oversee care plans for senior clients in home and community care settings
- Provide hands-on care and assistance with activities of daily living (ADLs)
- Manage behaviors and social interactions to ensure a safe and supportive environment
- Monitor vital signs and health status of senior clients
- Collaborate with healthcare professionals, families, and caregivers to optimize care plans
- Assist with lifting and mobility as needed
- Maintain accurate documentation of client care and progress
Qualifications
- Previous experience in caregiving or related fields such as social work or long-term care
- Knowledge of behavior management techniques for seniors
- Ability to provide physical assistance including lifting when necessary
- Familiarity with group home environments and care practices
- Proficient in conducting vital sign assessments
- Strong communication skills to interact effectively with seniors, families, and healthcare providers
Job Type: Part-time
Expected hours: 20 – 40 per week

Caregiving: 3 years (Required)
