Registered Nurse Case Manager

Job Summary As a Registered Nurse Case Manager at our facility, you will play a crucial role in coordinating patient care and providing support to patients and their families. You will work closely with healthcare providers to ensure the best possible outcomes for patients. Weekend Requirements - CM RN There are required to work a minimum of 4 weekend shifts in a 6 week period. Responsibilities Conduct patient assessments and develop individualized care plans Coordinate and monitor patient care services Collaborate with healthcare providers to optimize patient outcomes Provide patient education and support Utilize utilization management techniques to ensure cost-effective care delivery Work in various settings such as hospitals, clinics, and medical-surgical units Utilize your knowledge of physiology and hospital medicine to provide quality care Specialize in pediatrics care for young patients Experience Previous experience in patient service and case management is preferred Familiarity with Level I Trauma Center protocols is a plus Strong background in medical-surgical nursing Ability to work effectively in a fast-paced environment Excellent communication and interpersonal skills Job Type: Contract Pay: $70.00 per hour Expected hours: 40 per week Benefits: 401(k) 401(k) matching Dental insurance Health insurance Life insurance Paid time off Vision insurance Schedule: 8 hour shift Day shift Experience: RN: 1 year (Required) Case management: 1 year (Required) License/Certification: RN (Required) Ability to Commute: Apple Valley, CA 92307 (Required) Work Location: In person
