Registered Nurse (Pulmonary Transplant Care Coordinator)

The Lung Transplant Coordinator is an integral member of a larger interdisciplinary team contributing to excellence in patient care, research, teaching and in providing leadership to the organization. Mandated by the National VA Transplant Office, and guided by the Program Director, the Lung Transplant Coordinator is charged with the execution of the VACO transplant directive VHA 2001-027.The incumbent will maintain a multi-disciplinary program which provides comprehensive, cost-efficient lung transplant care to Veterans and their families.Duties include but are not limited to:Assisting Veterans, family members, and caregivers with receiving the most appropriate transplant options and services to meet their complex health care needs.Coordinating timely processing of pre/post transplantation patients care coordination requirements, to include repetitive record review to assure that ongoing monitoring and timely submission of transplant consideration packets into Veterans Affairs Central Office (VACO) Tracer Program, travel, and imaging/pathology.Coordinating care with multiple providers across all levels and sites of care. Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients.Cognitive decision-making, communication, and counseling to address the patients' needs.Continued monitoring of proper medication management, treatment plans, and team goals.Addressing psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team.Coordinating and expediting care from diagnosis to treatment.Analyzing and managing laboratory and radiology results and providing information to the providers.Utilizing databases to provide population management to track patient care through all episodes of care.Evaluating care and outcomes to ensure timely and appropriate provision of services.Advocating for holistic care through patient/family education and provider interaction and implements an educational plan to meet the patient care needs.Providing continuous patient education about transplantation status and future needs/requirements for successful transplantation.Developing and evaluating patient education materials to support Veterans and families throughout transplant process.Initiating and participating in Quality Improvement efforts to continually improve the transplant experience and outcomes.Evaluating the progress and success of the transplant program by maintaining and evaluating outcomes data.
