HOME CARE AGENCY is seeking: PHYSICAL THERAPISTS - To service clients in Nassau County, NY. FEE FOR SERVICE or FULL-TIME positions available in Nassau County, NY. Able Health Care is a Special Needs Certified Home Care Agency that provides Home Health Aides, Physical Therapists, Occupational Therapists and Speech Therapists to the homes of clients with developmental disabilities. We are seeking Fee for Service or Full-Time therapists to work independently and provide therapy services in our client’s homes. Perform patient assessment Select and adapt appropriate therapy treatment to maximize performance in activities of daily living Educate client, family & Home Health Aide of ways to approach tasks, as well as maintain safety Document progress report identifying treatment plan, observation, and goals. Skills: Valid license to practice therapy in New York State Knowledge of treatment practices per discipline in accordance with licensure. Excellent interpersonal communication skills 1 Year (preferred) of acute or home care experience.
