Job DescriptionThis position is PRN and works as needed based on program needs.Under the supervision of the Project Director – Williamson County Homeless Prevention (WCHP), the Homeless Prevention Crisis Clinician PRN provides client-centered support to individuals experiencing chronic homelessness in Williamson County. The primary objective of this position is to assist individuals in securing permanent housing, with a strong focus on placement through Community First! Village (CFV).This clinician engages individuals referred primarily by local law enforcement agencies, county jails, psychiatric hospitals, and community outreach. The role includes assessing eligibility and interest in housing programs, facilitating applications to Community First! Village, and connecting individuals to Supported Housing or other appropriate community-based resources when CFV is not desired or feasible.In collaboration with the WCHP multidisciplinary team, the clinician manages a caseload of up to 15 individuals housed at CFV and up to 10 individuals in alternate transitional or supportive housing programs. Responsibilities include wraparound care coordination and direct service delivery, such as linkage to clothing resources, medical and psychiatric care, substance use services, benefits acquisition (SSI/SSDI, SNAP, Medicaid), and other stabilization supports necessary to promote long-term housing retention.This position requires strong clinical judgment, crisis intervention skills, and the ability to build rapport with individuals experiencing complex trauma, behavioral health challenges, and co-occurring disorders. The clinician must demonstrate excellent written and verbal communication skills, including completion of accurate and timely documentation within an integrated electronic database system on the same day services are delivered.The ideal candidate demonstrates cultural humility, professionalism, and the ability to collaborate effectively with diverse community stakeholders, including law enforcement, hospitals, housing providers, and social service agencies. This role requires flexibility to triage tasks, organize daily priorities independently, and maintain productivity in both field-based and office settings. Evening and occasional weekend availability may be required to meet client needs.The Homeless Prevention Crisis Clinician PRN must be able to work autonomously while maintaining strong team collaboration to advance program goals and ensure continuity of care.Critical ImpactThe Homeless Prevention Crisis Clinician PRN plays a key role in reducing chronic homelessness in Williamson County by connecting high-risk individuals to permanent housing solutions. Through engagement, crisis response, and coordinated wraparound services, this position helps interrupt cycles of homelessness, incarceration, and hospitalization. By facilitating placement at Community First! Village and other supportive housing options, the clinician promotes long-term housing stability, reduces strain on emergency and public safety systems, and strengthens cross-system collaboration. This role directly supports improved individual outcomes, enhanced community well-being, and responsible stewardship of public resources.Key ResponsibilitiesProvide empathetic active listening, crisis intervention, and de-escalation to individuals experiencing behavioral health or housing instability crises.Conduct person-centered safety planning utilizing critical thinking and solution-focused problem-solving approaches.Ensure timely crisis follow-up and relapse prevention support to promote stabilization and reduce recurrence of crisis episodes.Communicate effectively and professionally with individuals and families from diverse cultural, linguistic, and socio-economic backgrounds.Facilitate and coordinate housing placements, including collaboration with Community First!
