In this role, you will partner with our team of APP's in the sub-acute rehab setting. Communication and collaboration with the patient's primary care team to ensure the best outcomes. Timely charting/documentation is required but can be done from home. Supervise nurse practitioners and physician assistants who are providing PM & R services in a sub-acute setting.
