Our platform automates charting and compliance so you can focus on high-quality senior care.Role OverviewWe're seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.What You'll DoRoutine & urgent assessments; chronic disease managementPost-discharge follow-ups, med reconciliation, labs/diagnosticsCoordinate with facility staff, specialists, families/POAsSame-day documentation; CMS/regulatory complianceSchedule & SettingFull-time position with a structured weekday schedule. You'll spend 3 days delivering on-site care across Stockton, Modesto, and Amador, with strategically clustered routes designed to minimize travel and maximize time with patients.
