You will lead the home care operation for our Bowling Green and surrounding-county service area, running the team that keeps older adults and people with disabilities safely at home and connected to their community. Bowling Green serves a diverse population that includes significant Bosnian, Burmese, Congolese, and Bhutanese communities alongside seniors who have called this region home for generations. It is how we operate.What you will doManage daily operations and staffing for the in-home attendant care team across Bowling Green and surrounding countiesRecruit, onboard, train, and supervise the attendant care workforceAssess each participant's home environment and care plan to confirm safety, individualized choice, and compliance with federal, state, and company standardsDevelop and update individualized home care service plansInvestigate and resolve any grievance or complaint regarding care deliveryPartner with referral sources, the Barren River Area Development District, hospital systems, and community organizations to grow census and protect qualityCoordinate with Freedom's compliance, case management, and adult day teams as one continuum of carePreferred qualificationsRegistered Nurse (RN) licensed in KentuckyPrevious experience in home health, home care, or HCBSFamiliarity with Kentucky Medicaid waiver programs (HCB, MPW, SCL)Bilingual or multilingual capabilityBachelor's degree in healthcare management, nursing, social work, or a related fieldBasic qualificationsLicensed Practical Nurse (LPN) in KentuckyAt least two years of experience working with seniors, individuals with intellectual and developmental disabilities, or bothLeadership and team management skills, including the ability to supervise, train, and support staffStrong written and verbal communication with participants, families, staff, and community partnersProficient with Microsoft Office Suite, scheduling, and documentation systemsWhy FreedomOne of the fastest growth, multi-state HCBC platforms with a clear career pathReal investment in our caregivers and our managers, because caregiving is real workDiverse participant population, diverse workforce, real community presenceMission that matters.
