With the support of more than 30,000 team members, we promote a forward-thinking philosophy built on a foundation of trust, dignity, respect, responsibility, and clinical excellence.Role OverviewAs a Hospital DME Liaison, you will serve as the primary point of coordination for patients being discharged with durable medical equipment (DME) and home oxygen. You will work closely with hospital staff, care coordination teams, and external partners to ensure timely delivery of equipment and a seamless discharge process.Key ResponsibilitiesServe as the main point of contact for hospital staff coordinating DME and home oxygen at dischargeCollaborate with Care Coordination and hospital teams to facilitate timely DME delivery prior to dischargeUtilize Cerner, HC+, and other EMR systems to manage and track patient ordersCoordinate with external DME vendors to ensure proper documentation and delivery of equipmentPartner with Customer Service teams to support timely order fulfillment and reduce delaysSupport seamless patient discharge workflows to help reduce readmissionsManage inventory for hospital DME supply areasMaintain clear communication between internal teams, vendors, and patientsPerform other duties as assignedQualifications🎓 Education –RequiredHigh School Diploma or GED🎓 Education –PreferredAssociate’s degree (in lieu of required experience)ExperienceRequired : 1 year of experience in a healthcare environmentOR 2 years of customer service experienceOR 6 months of experience with Cerner and HC+ systems🎓 Licensure –RequiredValid Florida Driver’s LicenseTotal Rewards & BenefitsBayCare Offers a Competitive Total Rewards Package, Including:Medical, Dental & Vision InsurancePaid Time Off (PTO)401(k) Match + Annual ContributionsTuition ReimbursementAnnual Performance Appraisals & Team Bonus ProgramCommunity discounts & wellness resourcesAdditional RequirementThis position requires Level II background screening through the Florida Care Provider Background Screening Clearinghouse (AHCA)Take the Next StepJoin BayCare and play a critical role in supporting safe, efficient patient discharges and continuity of care.👉 Apply today to become a Hospital DME Liaison!BayCare is an Equal Opportunity Employer.
