Employment Type:Part timeShift:Description:Position: Speech TherapistLocation: Enfield/Stafford AreaEmployment Type: Part-Time, 24 hours weeklyBenefits Start: Day 1Sign on Bonus: $2500Position Purpose:As a Speech Therapist at Trinity Health Of New England At Home, you will provide professional speech and audiology diagnostic and therapeutic services to agency clients as authorized by their physician.Key Responsibilities:Client Assessment & Planning:Accurately assess clients at admission and required time points using standardized documentation. Develop a treatment plan in collaboration with the physician and client, focusing on achievable goals to improve or stabilize the client's condition.Visit Prioritization:Effectively prioritize and plan visits based on client capabilities, condition, physician orders, and available resources.Regulatory Compliance:Maintain a working knowledge of Federal, State, and local laws and regulations, including the Organizational Integrity Program, Code of Ethics, and agency policies. Adhere to these standards with honesty, ethics, and professionalism.Care Coordination:Make appropriate referrals to other disciplines and services. Coordinate care to ensure effective and efficient service delivery.Intervention & Education:Implement preventive, therapeutic, and rehabilitative procedures according to the treatment plan.
