Home Health RN The Home Health RN provides skilled nursing services to patients in their homes, supporting recovery and long-term health management. This position requires independent clinical judgment, strong organizational skills, and a focus on delivering patient-centered care. Key Responsibilities Assess patient health status during home visits and identify care needs Develop and carry out individualized care plans in accordance with physician orders Provide nursing care such as medication administration, wound care, and monitoring of chronic conditions Track patient progress and adjust care as necessary Maintain clear, accurate, and timely clinical documentation Educate patients and caregivers on treatment plans, medications, and home care techniques Communicate effectively with physicians and other healthcare providers Promote safety and infection control within the home environment Qualifications Active Registered Nurse license Graduate of an accredited nursing program Current CPR/BLS certification Previous clinical experience required; home health experience preferred Strong critical thinking and assessment skills Ability to work independently and manage a caseload Reliable transportation for travel between patient homes Work Setting Patient care delivered in private homes Local travel required on a daily basis Flexible scheduling based on patient assignments Physical Requirements Ability to assist patients with mobility and daily care Frequent standing, walking, bending, and lifting Ability to safely handle medical equipment Benefits (optional) Flexible work schedule Mileage reimbursement Health insurance options Paid time off Opportunities for continuing education
