Senior Residence Counselor

Job Description Job Summary- Community Missions has (3) Senior Residence Counselor positions available in our adult division. The positions are responsible for the implementation of trauma informed and person-centered individualized service plans. In addition, they supervise daily activities and maintain a safe and supportive atmosphere for the residents. Senior Residence Counselors work closely with residents, their families, and a network of service providers to ensure the residents receive the necessary support to achieve their personal goals.Key Responsibilities-· Conduct assessments of residents' strengths and needs.· Develop and implement individualized care plans that address resident needs and goals· Monitor resident progress through regular follow-ups and adjust care plans as needed· Advocate for residents to ensure they receive appropriate services and support.· Maintain accurate and up-to-date case files and documentation in compliance with regulatory requirements and policies.· Assist residents with daily living activities, such as meal preparation, medication management, and cleaning.· Provide crisis intervention and support to residents in emergency situations· Ensure compliance with all relevant policies, procedures, and regulations· On-call responsibilities in rotation with a team· Hours include evening, weekend, and holidays.Qualifications-· Bachelors degree in psychology, social work, counseling, or a related field preferred; Associates Degree in related field plus1 year professional experience in the human services field· Valid New York State Driver's License with clean driving recordCompany DescriptionCommunity Missions of Niagara Frontier, Inc.
