Under general supervision of the Neighborhood Manager, Nurse Specialist or Registered Nurse, the LPN provides prescribed medical treatment and personal care services to residents. New Hiring Range base pay for LPN's: $31.19 - $36.15 per hour not including any applicable shift differentials based on experience. Brewster Village is accepting applications for full time, part time, and casual call positions in Long Term Care and Short Term Rehabilitation. The hours are 6:00am-2:30pm, 2:00pm-10:30pm, or 10:00pm-6:30am. A shift differential is offered as follows: PM Shift (2:00pm-10:30pm $3 per hour, NOC Shift 10:00pm-6:30am $2 per hour. All Weekend hours include a $2 Differential per hour.Key Responsibilities The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. Provides direct bedside care, and restorative care such as ambulation and range of motion. Administers medications and treatments as prescribed by the physician, assessing responses, documenting in medical records, and reporting adverse reactions or change of condition to Neighborhood Coordinator, Shift Coordinator, Physician, and family/guardian. Counts all controlled substances on unit with on-coming Staff Nurse at shift change, and destroys outdated and discontinued medications in a timely manner. Verbally communicates residents' acute conditions and instructions for follow through at shift change. Follows up on Twenty-four Hour Report of residents' acute conditions. Reports all infections to Infection Control Coordinator and sets up precautions as needed. Assists physician, as delegated by the Wellness Nurse-RN, during acute rounds, chart reviews and medical record reviews. Transcribes (or counter signs Health Records Clerk's transcription of) physicians' orders. Completes weekly nursing progress note on each SNF resident and bi-weekly note on each ICF resident. Responds to code situations, assessing residents' critical needs and taking proper steps to resolve the situation, under the direction of an RN. Acts as lead person, assigning and reviewing work of Nursing Assistant, and resolving work problems. Monitors residents during meals in dining room. Assembles and uses equipment such as catheters, tracheotomy tubes, oxygen suppliers, etc. Records food and fluid intake and output, and collects samples from residents for testing. Updates nursing care plans as delegated by the Wellness Nurse-RN. Assists with and participates in social activities of resident's choice which provide meaningful ways to spend time such as recreation, exercise, relationship building and fun. Supports maintaining a clean safe household environment. Respects the resident's room as home. Participates in team decisions and completes tasks as a team. Maintains regular and predictable attendance, works overtime/extra hours as required. Performs other duties as assigned. Education/Certifications/Experience Requirements Graduate from a Licensed Practical Nurse Program, State of Wisconsin Practical Nurse license required, nursing experience desired. Must provide current CPR Certification documentation prior to start date. Must maintain current certification in adult CPR. This position does require successful completion of a Post Offer Pre Employment Examination (POPE). Required or Preferred Skills Knowledge of the principles and practices of nursing care, mental illness, developmental disabilities, geriatrics and techniques of active treatment and behavior management. Demonstrated ability to provide quality nursing care. Knowledge of and demonstrated ability to conform to goals, policies, and procedures of the Department, Brewster Village and the Outagamie County Work Rules and Terms of Employment. Demonstrated ability to comply with appropriate safety regulations. Ability to delegate work assignments. Ability to observe and assess residents, recognizing unusual medication reactions. Knowledge of medications, side effects, dosages, mixtures, etc. Ability to communicate effectively with other departments, verbally and in writing. Ability to respond to code situations in compliance with training and licensure. Ability to assign and review the work of others in a timely manner. Ability to analyze and resolve work problems, and enforce policies and procedures. Ability to effectively communicate in English and understand and follow written and oral instructions. Long-Term Disability Insurance All regular, non-union full-time employees are eligible for long-term disability coverage. Premiums are fully paid by the County. Wisconsin Retirement System Wisconsin Retirement Program helps provide for financial security during retirement. Monthly annuity payments at retirement are calculated using years of creditable service, average earnings (based on three highest years of earnings), formula factors, age at retirement and selected annuity option. The retirement plan consists of two parts: an employee-required portion and an employer-required portion. Outagamie County pays the employer portion, and employees are responsible for paying the employee-required portion, which is determined by the state each year. Enrollment is required for eligible employees. Eligibility is determined based on Wisconsin Retirement System plan rules. Deferred Compensation Deferred Compensation allows employees to set aside pre-tax earnings toward retirement. Employees can save up to 100% of their compensation, to a maximum of $23,500 annually, or as little as $10.00 per paycheck. The amount deferred reduces year-end state and federal income tax base. All the earnings will accrue tax-deferred until such time as the assets are withdrawn, generally at retirement, termination, or other special circumstances. There is also an after-tax 457 Roth contribution option. Flexible Spending Account Plan The Flexible Spending Account Plan saves money by allowing the employee to pay for certain medical related expenses and dependent care expenses on a pre-tax basis, reducing annual taxable income resulting in Federal, State, Social Security, and Medicare tax savings. Employee Assistance Program Outagamie County has an Employee Assistance Program (EAP). The services offered as part of the County's plan are a benefit provided at no cost to its employees and their immediate family members to help deal with life's stresses.
