Restorative Nurse ( LPN ) $15k Sign On Bonus We are in search of Restorative LPN Nurse# The Restorative Nurse assumes the daily responsibility and accountability for the restorative aspect of the nursing care of all residents assigned.# Coordinates other personnel in the delivery of restorative care and services according to each resident's comprehensive resident assessment and plan of care.# Utilizes resources in an efficient and economic manner to ensure that each resident achieves the highest practicable physical, mental, and psychosocial well being.Confer with the Director of Nursing Services and/or licensed nursing personnel regarding restorative care of residents in the facility.Participates in MDS/RAI process and care planning of restorative care needs based on policies and procedures.Initiate requests for therapy/evaluation. Must respond to requests from the resident, physician or nursing staff. Be liaison between therapy and nursing department.Evaluate the resident and his/her records or charts, and discriminate between normal and abnormal findings in order to recognize when to refer the resident to a physician for evaluation, supervision or directions.Implement and maintain established policies and procedures relative to restorative care.
